Indications for the Need of Care Management Services

In considering whether a LifeCompass Care Manager may be able to help you or your loved one, take a minute to ask yourself the following questions:

Does the aging individual:

  • Live alone or are there no family members living close by?
  • Frequently miss medical appointments?
  • Show evidence of poor personal hygiene or poor nutrition?
