These Services Are Billed as Fee For Service
Comprehensive Assessment w/ Care Plan:
This is designed to look at all the factors that affect an elderly client’s health safety and quality of life. It includes an assessment of health history, psycho-social evaluation, functional evaluation, and a home environment safety analysis. Assessments are conducted with the client in the place of their residence and usually take two to three hours to complete and can be done in either one or two sessions. The individualized written Care Plan would include problem areas that were observed by the LifeCompassCare Manager and their recommended interventions and solutions.
Home Environment/Safety Assessment:
This includes an evaluation of safety factors (example: functioning smoke alarms, exposed wiring) and how affectively the older person is able to function within their home. Special attention is made to potential fall hazards and recommendations are made for installation of added safety features (grab bars, alarms on outside doors, raised toilet seats).
Facility Evaluation:
The facility assessment can include recommendationson the kind and level of care needed and provide information regarding quality and cost of various facilities. The LifeCompass Care Manager can also work with client to reduce resistance to moving help them with adjusting to the new environment and provide on-going project reports for priority members.
